Als fysiotherapeut is het van belang om op de hoogte te blijven van de meest actuele ontwikkelingen in het vakgebied. Hieronder valt ook het voortdurend groeiende aanbod aan evidentie vanuit de wetenschap. Onze docenten zijn continu op zoek naar interessante, actuele wetenschappelijke artikelen waarmee u uw kennis verrijkt. Daarnaast biedt dit u de mogelijkheid om uw therapieën empirisch te ondersteunen.

Graag helpen wij u hierbij door het tweewekelijks aanreiken van de meest actuele wetenschappelijke kennis uit de fysiotherapie. Deze week is onze docent Eugène Bakker gestuit op het volgende artikel:

‘Medial Plica Syndrome of the Knee: Diagnosis with Dynamic Sonography’ (Paczesny en Kruczyński, 2009)

Paczesny en Kruczyński besteden in dit artikel aandacht aan het maken van een diagnose met behulp van de dynamische echografische techniek. Deze techniek heeft een hoge mate van gevoeligheid en specificiteit bij de diagnose van de mediale plica en toont aan of er een interactie is met de patella en de mediale femurcondyl. Daarnaast reiken de onderzoekers een goede beschrijving van de echografische techniek aan en worden verschillende voorbeelden belicht.

Abstract

The role of medial plicae in disorders of the patellofemoral joint has been previously discussed, but many controversies on the importance of this remnant of embryological development in pathologic findings of the knee remain. In 1993, Johnson et al proved the efficiency of arthroscopic resection of the plica in the treatment of plica syndrome. Unfortunately, the signs of this syndrome overlap with signs of other pathologic findings of the knee. Common clinical symptoms such as aching, giving way, a dull sensation along the medial patellar border, or joint locking are not specific and can lead to misdiagnosis.

Magnetic resonance (MR) imaging has proved useful in detection of plicae but has some disadvantages. With static MR imaging, visualization of the presence of a medial plica is possible but the dynamic behavior of the medial plica cannot be examined. Other imaging techniques, including classic arthrography or CT arthrography, are invasive. Dynamic sonography, together with sonopalpation (tissue recognition under transducer pressure and pain perception when passing a transducer over the affected part), allows more accurate diagnosis of meniscal tears or ligament lesions. The aim of the study was to perform a feasibility study of dynamic sonography for the diagnosis of medial plica syndrome of the knee.
